Abstract

Definition penile fracture is defined as blunt trauma resulting in the rupture of the tunica albuginea of the corpus cavernosum during erection. The high frequency of penile fracture in the Mediterranean, is neither due to physical nor genetic particularities. It is due to sociocultural characteristics and lack of scientific knowledge about the genitals. Vigorous sexual intercourse was found to be the cause in 30–50% of cases in the West. Those from the Middle East resulted mainly from penile manipulations at masturbation. Diagnosis of penile fracture is mostly depended on clinical picture. Concomitant urethral injuries, presenting with hematuria, bleeding from the meatus, and voiding difficulties. Cavernosography is unnecessary for diagnosis of penile fracture and sonography can detect the exact site of the tear, associated urethral injuries may be difficult to detect.. MR imaging can accurately demonstrate the integrity of the tunica albuginea as well as the extent and location of a tunical tear and of associated injuries to the corpus spongiosum and urethra. MR imaging is also particularly helpful in determining the necessity for surgical intervention. Management conservative management is exceptional. It has higher complications rate, advocated only to uncomplicated cases with small heamatoma with no definite diagnosed tunical tear or limited to patients who reject surgery. Patients must be clearly informed on all these complications.Early surgical treatment is associated with a low incidence of late complications .Penile fracture is a true urologic emergency. It should be treated surgically as early as possible to ensure a better outcome.
