Abstract

Introduction: Coronary artery bypass grafting constitutes the keystone of adult cardiac surgery. CABG has classically been carried out through the median sternotomy approach using cardiopulmonary bypass with cardiac arrest. This method provides the most ideal operative field for a motionless and bloodless heart surface, with easy access to epicardial coronary arteries. Efforts were made to move to off pump CABG because of well documented side effects of cardiopulmonary bypass. Patients who underwent off pump CABG has been associated with lower morbidity and mortality and has similar graft patency rates to patients who underwent on pump CABG. Patients using the off pump technique, however, needed a shorter stay in hospital, shorter duration of mechanical ventilation in the intensive therapy unit, and fewer blood transfusions, and had better preservation of neurocognitive function
Aim of work: Aim of this study is to review the benefits and advantages of using off pump technique in coronary artery revascularization. It also gives an idea about the other operative techniques used in coronary artery bypass grafting.
Conclusions: The percentage of patients undergoing OPCABG continues to remain static at 20 to 25%. Advocates continue to feel strongly that significant benefit is afforded patients. The strongest evidence of benefit appears to be in patients at highest risk for complications. The data forthcoming in the foreseeable future are unlikely to shed further light on the extensive body of literature that has accumulated over the past decade.
