Abstract

64-section multidetector CT can provide accurate anatomical information, regarding the anatomy of the urinary tract and renal vasculature. It can detect renal and vascular abnormalities that might be potentially significant for safe renal donation. In addition, our new examination-perfusion CT- can give true idea about kidney function, that make advantage to MDCT as mentioned below:
• Addition of perfusion CT scan in the protocol of MDCT used in evaluation of living kidney donors makes it not only a perfect examination that allow excellent assessment of vascular, parenchymal and urothelial variants, but also can accurately determine the relative function of each kidney separately with no significant difference between it and the isotope scan.
So, a single MDCT examination of the donors allowing the proper selection of the kidney for nephrectomy based on both anatomical and functional assessment.
• Addition of perfusion CT scan in MDCT protocol used in evaluation of patients with renal masses will acts in two aspects:
( I ) perfusion CT values from the mass can be used in characterization of renal masses preoperatively instead of depending on the change of enhancement which face many pitfalls during its measurement.
( II ) perfusion CT values from the kidney obtained through the same scan can get reliable idea about the function of the remaining part of the parenchyma and this can alter the surgical decision especially if nephron sparing surgery was planned.
• In cases of unilateral obstruction submitted to under go CT examination, addition of perfusion study make CT not only able in determination the site, cause and the degree of obstruction, but also can give accurate idea about the function of this obstructed kidney without the need to re-examine the patients by isotope scan.
