Abstract

Study objectives: This work is aimed to compare the two design concepts of implant assisted RPOD. Material and methods: Ten partially edentulous male patients were selected to this study with completely edentulous maxillary arch against mandibular distal extension saddle. For all patients osseointegrated implant were installed distal to the first molar area. After osseointegration period patients were equally divided into two groups according to the two different RPOD design concept as follows; Group I: where healing abutment was used to provide only support for saddle of the RPOD. Group II: where ball abutment and matrix was used to provide only retention of the RPOD. The mean marginal bone loss (MBL) was measured on panoramic radiograph immediately before, 6 and 12 month of denture insertion. Data was collected and statistically analyzed. Results revealed that: 1- The mean tooth MBL in patients wearing implant supported or retained RPODs was significant after first six months, second six months and one year of denture insertion. 2- The mean tooth MBL in patients wearing implant supported or retained RPODs was insignificant between both groups for all periods of study. 3- The mean implant MBL in patients of implant supported or retained RPODs was significant after first six months, second six months and one year of denture insertion. 4- The mean implant MBL in patients of implant supported or retained RPODs was insignificant after first 6 months and 12 months of denture insertion. But it was significant difference in between groups after the second six months of denture insertion from the results of this study, it could be concluded that: 5- The mean implant and distal tooth abutment marginal bone heights were reduced after one year of denture insertion regardless the implant RPOD design concept. 6- Maintenance of implant RPOD designs must be started six months after insertion. Recommendation: 7- Prognosis of teeth and implant abutments are not promising within the short period of the study, a longitudinal study is recommended to monitor abutments and ridge.
