Abstract

Hepatitis (HCV) infects an estimated 170 million persons world wide with the highest number of infection reported in Egypt. Progression to chronic hepatitis occurs in the majority of HCV infected persons.Liver cirrhosis is the end stage of chronic liver disease. Cirrhosis is characterized by diffuse fibrosis followed by nodular parenchymal regeneration and subsequently loss of lobular structure due to accumulation of ECM through activation of many cells including Hepatic stellate cells which is the most activator for fibrogenisis.Cirrhosis is initiated by hepatic injury like viral infection. Injured hepatocytes produce apoptotic bodies and free radicals besides inflammatory cytokines and chemokines released by inflammatory cells. All these molecules stimulate Hepatic stellate cells (HSC) which dominate the progression of hepatic fibinrogenesis.Interferon (INF) based antiviral therapies have been adopted for treating chronic HCV. Currently INF-alpha or pegylated- INF- alpha in combination with Ribavirin (RBV) therapy is the most common therapy for chronic HCV. Many studies have been done showing role of stem cell transplantation in liver cirrhosis due to HCV and other causes. A clinical trial on 9 patients with decompensated liver cirrhosis due to causes other than HCV was implemented and significant improvements were observed. Another study was done on four patients. The result was good as regard two of the four.
