Abstract General anesthesia is currently the standard technique used for anesthesia of breast cancer .However , it is accompanied with side effects and complications .In this study we aimed to assess safety and efficacy of the regional anesthetic techniques thoracic epidural and thoracic Para vertebral block for operative management of breast cancer and to compare postoperative pain ,shoulder movement ,nausea , vomiting and hemodynamic state in patients undergoing unilateral breast surgery using the three techniques . Ninty patients undergoing unilateral breast surgery were randomly classified into three groups. 30 patients received general anesthesia, 30 patients undergoing surgery using continuous thoracic epidural anesthesia and 30 patients used continuous thoracic Para vertebral for the operation .Heart rate , mean blood pressure , pain score (VAS) ,restricted shoulder movement and any side effects , if any were recorded every 2 hours in first 12 hours then every 4 hours in next 12 hours postoperatively .When pain score was ≥4 analgesia was given in the form of IM pethidine in GA group and extra dose of local anesthetics solution in the two regional groups . The study reveled that the surgery was successfully completed in majority of cases except 6 cases in regional groups in whom the block failed and excluded from the study .Pain scores was less in PVB group than the other 2 groups (p≤0.05), also restricted shoulder movement was less in both regional groups. Postoperative analgesic consumption and PONV is less in PVB group when compared to other two groups. Patient satisfaction is comparable in three groups. |
