Abstract As cataract surgery is considered a refractive procedure, so it is important to take care of the refractive outcome of the procedure. For this reason several parameters should be taken into consideration including biometry, type of IOL used, technique of cataract surgery and correction of any residual postoperative refractive error. Accurate axial length measurement, by A-scan ultrasound or optical coherence biometry or MRI, and topography can be used in irregular astigmatism, keratometry and appropriate choice of the IOL calculation. For eyes with previous refractive surgery there are indirect methods, topographic method, pachymetric method, corneal radius correcting factor and measurement of anterior and posterior corneal power using either Scheimpflug corneal imaging or using Orbscan corneal imaging. The choice of IOLs includes toric IOL and presbyopia correcting IOLs, aspheric, multifocal or accommodating. Residual error after cataract can be corrected by LASIK, PRK or conductive keratoplasty. However, the potential need for these procedures should be discussed with the patient in advance, to make sure the patient understands, and to reach maximum satisfaction. |
