Abstract

In view of the increased risk of myocardial ischaemia in a population with carotid atherosclerosis considering atherosclrosis a progressiv disease which, when advanced involves multiple sites. and the low prevalence of positive exercise ECG responses in hyperlipidemic patients, we decided to determine whether the presence of carotid atherosclerosis is predictive of a positive exercise ECG in asymptomatic hyperlipidernic patients. Out of 234 patients referred to our Cardiology department, Tanta University Hospital between January and December 1993, 81 subjects fulfil our inclusion criteria being asymptomatic hyperlipidemic with or without other risk factors for CAD. Their ages ranged from 28-66 ( with a mean of 46 + 9.3 years), 60 patients were males and 21 were females. The main clinical and laboratory data for each subject were studied carefully including the major risk factors for CAD (hyperlipidemia, smoking, diabetes mellitus, hypertension, and family history of CAD). All our patients were subjected to a maximal exercise test adopted from Bruce protocol using a commercially available machine (Marquette Max I). Every patient was asked to give a true maximal effort. A test was considered positive if the ST segment was displaced by > 1 mm horizontal or downsloping depression at 80 ms from the J- point.
