Abstract

waa to t r y t o throw more light on the nature, characterization and mechanism of appearance of protein in the urine under both physiologic and pathologic conditions. ’It alao aimed to find, if there is a characteristic pattern of protein excretion, or a charac t e r i s t i c pattern of selectivity of proteinuria, in one condition o r another, which may be used as a clue to differential diagnoaia of proteinuria. For this purpose a t o t a l number of 111 cases ranging from 3 years to 69 years, were carefully selected from the attendants of the inpatient of Tanta University Hospital. These included 24 apparently healthy subjects (14 adult & 10 children) taken as reference group; 12 healthy subj scta proved to have proteinuria after strenuous exercise; 12 women in the l a s t trimester of normal pregnancy ; 12 pregnant women having preeclmpsia; 13 children suffering from acute glomerulonephritis, 5 of them were examined during the first days of the disease while they have gross hematuria, the other 8 cclses were examined 5 to 10 days l a t e r where only microaoopic hematuria and mild proteinuria were still detected; 12 patients with chronic nephritis; 14 pat i e n t ~p resented with nephrotic lsyndrome, and 1 2 diabet ic patients, A l l these cases, except those of the reference group, proved to have prot einuria, but of dif f ereat degrees. They a l l were carefully subjected to clinical. and laboratory examinations to confirm the diagnosis and to assess the degree of kidney impairment. The study was conducted according to the following scheme : - 1- Different chemical and immunochemical studies were done for the 8 groups including, estimationstion of the tot a l proteins in the serum and urine using biuret method, eat-imation of serum albumin by the dye - binding method, electrophoretic separation of both serum and urine protein using cellulose acetate, quantitative detemination of different specific proteins .h aerum and urlae using the Laurel1 technique of imm~11oeleotrophoresis(l966) and quantit atfve estimation of immunoglobulin in serum and urine, uafng the radial immunodiffusion method of Mancini (1965). 2- This was followed by the calculation of the renal clearance of the atudied 9 proteins individually and comparfng the mean values of each group with the corresponding reference group. 3- The next step was the calculation of the selectivity index for every studied case using the formula estsblished by Jone and Hua (1980), and a trial was made to correlate t h i s selectivity index with the severity of the renal disease as judged by the serum creatinine, blood urea and creatinine clearance, and with the ramount of protein excreted per day. 4- Then an attempt was made to standardize the values of protein clearance, in a t r i a l to find out a characteristic pattern for one group o r another or a differentiating point between the physiologic md pathologic, i f not between all, forms of proteinuria.
