Abstract

Leprosy c o n s t i t u t e s a big s o c i a l wide world problem, problem f o r the humanity as it causes l o s s o f sensation aad marked mutilations. A t the same time it is an economic problem as it leads t o s i g n i f i c a n t l o s s of man power i n the countries where the disease is endemic. I n Egypt, there are 28.611 lepers on whom a proper sys-bematic study has so f a r not yet been conducted. The aim of this work was planned t o explore c e r t a i n bioohemical and hietopathological changes tvELh’,ch mcy accompany the cliacal picture of the disease. The subjects of this study,included 70 cases; 50 clinical oases of leprosy collected i n a non selected manner and 20 persons served as reference controls. Every case was subjected t o urine and s t o o l examination i n addition t o a knowledge of the p a s t h i s t o r y i n order t o exclude any b i l h a r z i a l affection. After thorough clinical and histopathological examination of skin biopsy, p a t i e n t s were c l a s s i f i e d as lepromatous lepers (28 cases) and tuberculoid lepers (22 cases), each group was f u r t h e r subdivided i n t o three subgroups, recent cases without treatment, cases under treatment and without mutilations and cases under.treatment but w i t h mutilations. Liver biopsy was done f o r a l l cases and the histopathological findings were correlated w i t h l i v e r function as estimated by a battery of t e s t s including q u a l i t a t i v e and quantitative changes i n plasma proteins, thymol turbidity, zink sulphate t u r b i d i t y , serum enzymes (SGP-T, SGO-T and alkaline phosphatase) ; Serum t o t a l cholesterol and blood sugar l e v e l s ( f a s t i n g and post prandial) . Electrocardiographic picture was done for all cases, t h e l e v e l s of corrected Q-T i n t e r v a l were correlated w i t h serum e l e c t r o l y t e s (calcium, phosphorus, sodium and potassium). Skin biopsies for a l l cases were stained with haematoxilin and eosin t o confirm the c l i n i c a l diagnosis and c l a s s i f i c a t i o n . Modified Bielshowskyts s t a i n was used t o v i s u a l i s e t h e cutaneous nerve terminals i n skin lesions i n lepromatous nodules compared w i t h tuberculoid lesions and normal contr o l s of the same age and sex.
