Abstract

’.This study was carried on 40 patients attending .inconsequence the Neuropsychiatry department,all of them were sufferring from manifestations of acute CVD which was confirmed by brain CT within the first 24 hours from admission with clinical. assessment for one month and re-CT study at tho end of that month. The patients were grouped into 3 groups,the first was the haernorrhagic group of 17 patients,the mean age of them was 55.12 years,the second was the embolic group of 6 patients with the mean age of 39.83 years and the third was the thrombotic group and their mean age was 57.94 years.Females(65.2%)were more common than rnales(34.8%)i.n occlusive strokes.while in haemorrhagic strokes males(64.7%)were more prone than females(35.3%). Hypertension was the most important encountered risk factor in our study,where 14 patients(82.4X)j.n the haemorrhagic and 11 patients(64.8%)in the thrombotic groups were hypertensive.On the other hand none of the embolic patients were hypertensive.Both systolic and diastolic readings of the haemor rhagic patients were statistically higher than those of the thrombotic group(t=2.87 and 2.4 consequently,p<O.O5).Diabetes mellitus was encountered in 12 haemorrhagic patients(70.5%),13 thrombotics(76.4%)and 2 embolics(33.3%).Smoking habit was practiced among (56%) of male patients in both haemorrhagic and thrombotic groups.Isch~emic heart disease w’as evident in 4 patients(23.5g)in both haemorrhagic and t.hrombotic groups and all the six embolic patients were sufferring from cardiac dysrrhythmias.Hyper1ipidaemia was found in(14.7%)of patients of haemorrhagic and thrombotic groups.Emotionai stress just before the insult was present in 10 patients(29.4g). Obesity was more common in thrombotic patients(4l%)than haemorrhagic ones(ll.8%). Onset headache was common in haemorrhagic than occlusive strokes-It was reported to happen in 9 patients(64g)of the haemorrhagic group,2 embolic patients(33.3g)and 1 thrombotic patient(5.8%).Sentinel or premonitory headache was encountered only among the thrombotic patients(lg%).The character of the headache in haemorrhagic stroke was mainly bursting and bilatera1,while that of occlusive strokes was mainly dull aching of mild degree and contralateral to the side of the lesion. Onset vomiting was more common in haemorrhagic than thrombotic patients(47.1% and 17.6% respectively),and it happened in 50% of embolic patients.The triade of headache,vomiting,and disturbed consciousness was found in 7 patients.4 of them(28.6%) were haemorrahgic and 3 were occlusive.
