Abstract

Endothelin-1 is an extremely potent vasoconstrictor of vascular smooth muscle, consisted of 21 amino acids, containing two intra-chain disulphide bridges, linking paired cysteine amino acid residues.Endothelin is not stored in subcellular vesicles. ?his makes it unlikely that it could arise from lysed endothelial cell within the infarct,but stimulated endothelial cells in the border zone of the infarct are possible source. Another possibility is that the endothelin was produced in extracardiac sites as a par$ of systemic response to myocardial infarction, perhaps secondary to the hypercoagulability and platelets hyperactivity seen in AM1 or to increased sympathetic activity.The aim of this work is to study the level of endothelin-1 in ischemic heart disease patients and determine their relation to cardiogenic enzymes (CK, LDH and AST) to throw more light on the role of endothelin- 1 in pathogenesis of AMI.In this work endothelin-1 was determined in 30 patients with ischemic heart disease, 5 patients with stable angina, 12 patients with unstable angina and 13 patients with AM1 comparing them to a reference group of 7 apparently normal subjects.
