Abstract

Sciatica is defined as pain radiating from back into buttocks and into the posterior andlor lateral aspect of leg. Now lumbar disc herniation is the most common cause of sciatica . The aim of this work is to compare .between epidural injection of corticosteroid with local anesthetic in the lower sacral hiatus and interferential current in treatment of sciatica . This study was carried out on 30 patient suffereing from symptoms and signs of unilateral L5 or S1 radiculopathy . They were collected from the out patient clinic of physical medicine & rehabilitation departement, Tanta University Hospital . Their age was ranged from 22 to 55 years with a mean age 4 1.8 f 7.7 years of whom 15 were inales and 15 were females, 16 patients were right sided and 14 were left sided . The patients were divided according the line of treatment into two groups matched in age , Sex and duration of the complaint . group (I) 111cluded 15 patients their ages ranged from 33-55 years with a mean age of 42 + 6.6 years of whom 8 were males and 7 were females. 7 Patients were right sided and 8 patients were left sided . 9 patients had a disc lesion between L4-5 and 6 patients had a disc between L5-S1 . 9 patients were manual workers, 4 patients were farmers and 2 patients were employers . ’The duration of the disease was ranged from 1 to 6 months with a mean duration 3.6 f 1,5 inontl~s. Summarv and Conclusion group (11) Included 15 patients their ages ranged from 25-55 years with a mean age 4 1.6 f 9 years of whom 7 patients were males and 8 were females. 9 Patients were right sided and 6 patients were left sided . 9 patients bad adisc lesion between L4-5 and 6 patients had a disc between L5-S1 . 10 patients were manual worlters, 4 patients were farmers and 1 patient was employer . The duration of the disease was ranged from 1 to 6 months with a mean duration of 3.3 rfi 1.5 months. from the results of this study we can concluded that epidural injection of the corticosteroids with local anesthetic are more effective in treatment of sciatica than interferential current and considered the simplest , safest and the most efective method in treatment of selected cases of sciatica . The resulits of this study could be summarized as follow : 1-There was significant improvement of pain score in group (I) (P < 0.05) and highly significant improvement in group (I1 ) (P < 0.001) . 2- The improvement of pain score was statistically insignificant regarding rest pain (P > 0.05) and was statistically significant regarding activity pain between group (I) and group (11) (P < 0.05) . 3- There was significant improvement of S.L.R.T. in group (1) (P < .05) and highly significant improvement of S.L.R.T. in group (11) (P< .001). 4- The improvement of S.L.R.T. in group (11) was statistically significant compared to the improvement of S.L.R.T. In group (I) (P < 0.05) . Summarv and Conclusion from the results of this work we can concluded that : 1) Both interferential current and epidural injection of corticosteroid with local anesthetic are effective in treatment of sciatica with superiority to epidural injection . 2) Epidural injection of corticosteroid with local anesthetic considered the simplest, safest and the most effective method in treatment of selected cases of sciatica .
