Abstract

CHOP (cyclophosphamide, Doxorubicin, Oncovine, and Prednisone) has been considered standard therapy for treatment of NHL. The present study aims at evaluation of Tissue Doppler Imaging in early detection of Adriamycin cardiotoxicity in NHL patients presented to our oncology department Menoufiya University and treated with CHOP regimen. The inclusion criteria were patients diagnosed as NHL by tissue biopsy with stage 1 to stage 4 of the disease and planned to receive CHOP regimen. Cardiac evaluation was done by chest x-ray, ECG, M-mode,two dimensional echocardiography & Tissue Doppler Imaging. 39 patients were evaluated for cardiotoxicity among these patients 5 patients developed signs of cardiac toxicity in the form of decreased EF these patients were classified as (group 1) and patients with normal EF were classified as ( group 2) . With conventional echo Doppler at base line, three patients out of 5 in [groupl] had EIA ratio< 1 & 13 patients out of 34 in [ group 21 had E/A ratio <1 Repeated measurements (before TTT, before 2nd cycle, before 4” cycle & after 6th cycle) revealed no significant changes in EIA ratio in both groups and comparing between both groups also revealed no significant changes. In contrast with TDI, A wave was significantly increased in mid anterior & basal anterior segments after the 6th cycle, with earlier increase of A wave occurred at mid lateral segment before the 4th cycle. E/A ratio was significantly reduced at mid anterior & basal anterior segments after the 6th cycle with early reduction before the 4th cycle at mid lateral segment, IMT was significantly prolonged in basal septal, basal lateral, basal anterior, basal inferior & mid inferior segments after the 6th cycle. This proves the superiority of TDI indices of diastolic function in early detection of Adriamycin cardiotoxicity. As regard systolic function measured by conventional echocardiography there was significant reduction of EF after the 6th cycle in [group 11 &no significant changes in EF in [group 21. with TDI global LV function assessed by longitudinal axis function was significantly decreased in [group 11 after the 6th cycle .and as regard regional hnctions, peak systolic velocities in group 1 were significantly decreased in basal& mid septal, mid lateral ,basal &mid anterior, basal& mid inferior.
