Abstract

study presented here is the first systematic comparative study of telomerase activity in pleural fluid and sputum that detect the level of hTERT -mRNA expression by quantitative multiplex realtime PCR
hTERT -mRNA expression is indicator for telomcrase enzyme activity. The use of (QR-T) real-time PCR method detect telomerase hTERT catalytic subunit quantitatively and overcome the conventional RT-PCR artifacts and avoids excessive handling of the samples.
Tclomerase activity may be useful both as a diagnostic marker to detect the existence of immortal lung cancer cells in pleural fluids and in sputum as a target for therapeutic intervention. therapeutic intervention can be directed at RNA component of human Tclomerase and the data show that SCLC may be a prime target for such therapes.
Telomerase expression and pattern are distinctive among the histopathological classes of lung cancer and could convey prognostic influence.
Weak or moderate telomerase activity was found in lymphocytes in some tuberculous pleural effusions and may explain the low specificity for malignancy some times obtained with the QRTPCR method. So we recommend exclusion of cases with manifest lymphcytosis or T.B. to increase the test specificity.
Our findings indicate that telomerase is a specific marker for malignant lung disease and can complement cytology and histopathology in the diagnosis of MPE due to lung canse. Since it is an intracellular marker, its activity can be estimated before
