Abstract This work was conducted to evaluate the effect of telescopic partial denture and bar retained over dentures on the supporting oral structures and compare these results with the theoretical results obtained from finite element stress analysis. For this purpose twenty patients were selected and divided into two equal groups. They had an edentulous upper jaw, and a canine and a first molar on each side of the lower jaw. For the first group, a telescopic partial denture that was placed over metallic copings was constructed while for the second group, an over denture retained by a laboratory bar unit was made. These two types were evaluated according to their effect on the gingiva by the gingival index, periodontal ligament by the pocket depth, the bone level of the abutments by periodical radiograph , and the edentulous segments of the ridge by panoramic radiographs. All these tests were repeated along 24 months to evaluate the changes of the oral structures included in this study. The theoretical results obtained were through the use of two dimensional finite element stress analysis by the location and magnitude of stresses resulting in the two mathematical models representing the two groups. The results of this study for both groups showed a significant increase in the gingival index, pocket depth, and loss of fiber attachment, and a significant decrease in the marginal bone level of the abutments. No significant difference was found for the edentulous segments between the initial measurements and those taken two years later. |
