Abstract

splenectomy is used as a treatment for portal hypertension patients with chronic liver disease and hypersplenism. The haemodynamic changes may play a role in increasing the risk of portal vein thrombosis which is a usual complication that occurs postsplenectomy .The haemodynamics changes in the portal system was studied using colour coded Duplex ultrasound in patients with portal hypertension preoperatively and postoperatively in order to discover the relation between the changes that occurred postsplenectomy and the incidence of P.V.T. Also clinical & laboratory investigations follow up were done for all the patients (40 patients).There were significant decrease in. the percentage change in the C.I. of the portal vein, the diameter of the portal vein, the velocity of the portal flow and the portal flow rate postsplenectomy which may be due to loss of part of the circulation (40%) due to splenectomy.
The incidence of the portal vein thrombosis was found to be 42.5%.lt is a high incidence which may be attributed to the cause of splenectomy (portal hypertension), the use of anticoagulants as well as the small number of patients .P.V.T. is a complication of splenectomized patients which is a difficult to be diagnosed clinically & it should be suspected in patients with pain and fever,
