Abstract Our work aimed to study the portal, splenic and superior mesenteric veins and splenic, hepatic arteries as regards flow volume, velocity and presence or absence of thrombosis by duplex pre and post Warren operation, 10 patients admitted to the haematemsis Unit in Alexandria Main University Hospital in a state of active bleeding due to portal hypertension are assessed by Dcpler ultrasonography pre and post DSRS. Accurate patient evaluation is essential to appropriate patient for DSRS. This depends on endoscopy for oesophageal varices grading and detection of risk factors of rebleeding, Duplex study for assessment of patency of splenic vein, flow volume, velocity and also for portal, superior mesenteric veins and hepatic, splenic arteries, anatomy and position of the left renal vein.
