Abstract - 68 - SUMMARY The ~.S.R. is a non specific test which is increased in a variety of diseases. The E.B.R. is increased in all conditions where there is tissue breakdown or where fore ign protein enters the circulation. Chronic tonsillitis is the most common of all recurrent throat diseases. Our study aimed at studying the changes of E.S.R. in patients with chronic tonsillitis before and after tonsillectomy. This work was conducted on 30 patients (16 males and 14 females) with chronic tonsillitis. For all patients the following were done: _ Thorough history taking. _ Thorough clinical examinations. _ Laboratory investigations : • Urine analysis. • Stool analysis. • Blood picture ~~nemoglobin fJercentage. • Bleeding time and clotting time. · ~.S.~.(Westergren method): _ 1”t hour J 2nd hour (before opar-a t L.on , 1 week, 2 weeks and aIlemonth after operation). Our cases were classified into three groups according to ----~ --~- --_ .. ---- ------- --_ .. -- - ------ ---_ .. -_.-- - 69 - the aetiological dia~nosis. The first group: Filtients with remote complications of chronic tonsillitis (rheumatic fever, post streptococcal glomerulonephritis). This group comprises 5 patients. The second group: Patients with parasite infestation. This group comprises 5 patients. The third group: Patients free from parasite infestation, rheumatic fever or post streptococcal glomerulonephritis. This group comprises 20 patients. In the first group, there is significant increase in E.S.R. ( 1st and 2nd hours) after 1 week, 2 weeks and one month (F<:o.05), the significant increase in the first week may be due to subclinicRl infection, effect of drugs and tissue tr-numa of surgery. where as after one month the E.S.H. sti- 11 shows significant increase, when this results were compared with those obtained in the third group, it is very clear that the cause of >Jersistent increase in B.S.H. is the complications of chronic tonsillitis. In the second group, t . ( st 2nd here ~s ~ns~~~f~cflnt ~ncrease ~n E.S.R. 1 and hours) after 1 week, 2 weeks and one month (P~o.05), in this group the chronic tonsillitis was the determining factor tlmt affects ~.3.H. before operation, but one month after operation E.S.H. shows minor changes inspite of tonsillectomy, SO another factor must be present and affects E.S.H. rhis factor is most probably the parasitic infestation. -~- -- -~----~- - _ .. _. - - -- - 70 - In the third group, there is significant increase in the i’;.S.H. ( 1st .md 2nd hours) in the first week, this is most probably due to the same causes as in the first group. In the second week, there is significant increase in E.S.H. in the first hour (p<:o.OS), while the second hour showed insignificant increase (1;>0.05). The cause of this increase in E.S.H. is the same causes of highly E.S.R. in the first week. After one month, there is insignificant increase in !C.S.H. (P>O.05). ----- --------- --- ----- -------------- |
