Abstract

Pregnancy induced hypertension is a disease of late pregnancy with increased maternal and fetal
morbidity and mortality.
An abnurmali ty of the placenta with abnormal
cytotrophoblast differentiation has been lmpl icated as
the underlying cause of pregnancy induced
hypert(’n si on,
The failure of rrophoblast ill vagi on and absence
of the phyainl ogie al adaptation of the spiral arteries
has been regarded as responsible for placental
pathological changes and the reduced utero placental
blood flow in pregnancy induced hypertension.
Studying of the umbilical artery blood flow
ve locimetry ami vascular resistance as determined by
Doppler hove been related. to placental micro-
VIISCU latur e.
An abnormal umbilical artery waveforms
reflected increased vascular resistance caused by
inadequate physiological changes in the sp iral
arteries which was asso ciated with an obliteration of
the srnul l arteries of the tertiary villi of the pl~centa.
Maternal hypertension is associated with high
res istance ~n the umb iJical artery wi th sign j ficant
increase in Doppler indices.
The 111m of this work was to study the pattern of
umbil ical artery Doppler ve locirnctry In cases
cornpl icate d by pregnancy induced hypertension and
norm oterisi vc cases .and compared with the pi acen tal
pathology of these cuse s.
The study included twenty women with pregnancy
induced hypertension and twenty normotensive women
as a control in rhe third trimester of pregnancy. They
were subjected to complete history, general, obstetrical
examination, laboratory investigations and ultrasound
examination.
Duppl er study of the umbilical artery blood now
ve locimctry was done using continuous motle of
Dop pler, After de livery the placenta was taken and
subjected to pathological examination,
In this study there was significant increase in
the degree of hypertension and proteinuria between
cases and control.
There was signi Iic an t mer ease III Doppler
indices (P<O.O 5) in the hypertens ive cast: 5 than the
control.
There was aiguificaut mcrease III Doppler
indices (SiD, Pulsatility index and pourcelot index)
with. increasing degree of hypertension and
proteinuria.
