Abstract

Dealing with cases of chronic sinusitis represents
a clinical problem. Anterior ethmoid sinus is the key
to chronic infections involving the maxillary sinus.
Persistence of infect in in the antenior ethmoid hinder
mucociliary clearance of the maxillary sinus through
its natural ostium.
Nasal endoscopy provides an excellent tool for
assessing the cause of drainage obstruction whether it
is a pathological abnormality or an anatomical one.
Nasal endcopy is an easy and accurate diagnostic
procedure. It can be done under local anaesthesia in
the out-patient clinic.

Endoscopic endonasal surgery of the diseased
ethmoids should be considered as an important element
in treating cases of chronic sinusitis.
This work included 50 patients suffering from
chronic maxillary sinusitis secondary to chronic
ethmoiditis. Maxillary sinusitis was confermed by trans
canine maxillary sinuscopy. Pre-operative C.T. Scaning
was performed to reveal any mucosal change deeper in
the osteomeatal area, to identify the extent of the
disease and also to detail the anatomy.
The main symptoms were post. nasal dischange
(90\), headache (82\), anterior nasal discharge (40\)
and nasal obstruction (24\).
During the follow up period the patients were
observed for subjective improvement in symptoms maximum
improvement was noticed in headache (95.12%) then nasal
obstructive (91.6%), anterior nasal discharge (75%),
with least improvement in post nasal discharge (66.7%).
