Abstract

The peripheral nerve blocks are used for:1- Anesthesia 2- Postoperative analgesia.3Diagnosis and treatment of chronic pain syndromes.Advantages and disadvantages of peripheral nerve blocks for anesthesia must be considered when advising patient about the choice of anesthesia . Sedation-analgesia should be advocated during block placement to improve patient acceptance . A peripheral nerve block of the lower extremity provides anesthesia with much less sympathectomy so no major haemodynamic changes similar to spinal or epidural block will occur . The lower extremity is supplied by nerves that are widely separated from each other as they enter the thigh. Major nerves to the lower extremity include the sciatic, femoral, lateral femoral cutaneous and obturator nerves. The sciatic nerve divides in the popliteal fossa into the tibial nerve passing medially and the common peroneal nerve passing laterally . The sciatic nerve can be blocked at different level providing excellent surgical and postoperative anesthesia and analgesia .The 3 in 1 block is a simple technique to block the femoral, lateral cutaneous nerve of the thigh and obturator nerve. This has been used successfully for surgery on or above the knee .Blockade of the sciatic nerve in the popliteal fossa provides satisfactory analgesia for operative procedures of the foot .
