SUMMARY
The concept ”Hospital information system” has by convention, become restricted to computerized systems. However, all repositories are information systems and every documentation is an information source. The increased flood of hospital records, which has been noticed in the last few years was the reason for the development of methods to extract the information contained there as completely and economically as possible. The goal of this extraction and its subsequent storage procedure is to retrieve the information fast and accurately.
The major step in hospital information system will be in their linking with the actual practice of medicine and to the continuation of the medical education.
The medical records, which constitute the fundamental and permanent basis of the medical information system, are now integral part of any medical establishment for both its day-to-day operations as well as decision support and future planning.
Design of the medical record automation formation, which is a very important step to create the link between the paper system and automated system. It is used for data entry to the computer data base system.
This study was done on the clinical data of the admitted cases in the Urology Department Benha Faculty of Medicine during the

period from Jan. 2001, till Dec. 2001. There were 508 patients admitted to the department for first time, twenty of them admitted for second visit and three of them admitted for third visit.
All computer programs found in the market are financial or administrative. So, we design a scientific computer based case sheet, which include all data of the patient and operative procedures done at the department. The program was designed using visual basic computer language. The data were consequently extend into the personal and clinical sheets and included into the computer program.
A property designed manual system must go hand in hand with the automated system especially in the transitional period.
The automated medical file is the final goal of this study. A database file is generated in the automated system, containing records for every patient. This data base file is the base line for the programs generated. The records contain information drived from the medical record automation format, used for data entry.
A successful record system should take into consideration managerial aspects. An appropriate space, allocation resources in the form of hardware and software, trained manpower with specific job description and clear protocols for work are needed.
Experience in our computer-based file keeping system indicates the following:
(1) Its function exceeds that of a repository for the clinical data. It is an important tool in the following areas:

A-Direct patient care: it provides a physician with a concise summary of the significant medical events in a patient’s past experience with the hospital.
B-Administration: It provides the hospital administration with information regarding the patient problems, the duration of hospital stay, the hospital delay... etc.
C-Education: It provides the residents with a well-structured and easily read picture of patient health status.
D-Research: Data analysis was done by the program which is very important in research purposes for example; searching by (Name, Code, State, Referral, Age, Marital state, Group, Section, Complaint diagnosis, treatment, operations done during certain period and operations done by certain surgeon.
(2)To allow easy computing the processable data within these records should be as minimum as possible.
(3)Templates must be prepared for selection of fixed terms to avoid errors of computer.
(4)Continuous system evaluation is the only way to avoid implementing a dead system.
(5)The questionnaire approach is the most reliable method to get standardized information for evaluation. It’s useful for a research of precise objectives, of certain time of application and of prospective nature.
The results were obtained; the program not only has the ability for documentation but also for searching different data as well.
