Abstract Tonsillectomy with BSG and epinephrine paste as a haemostatic method is new developingtechnique. The components ofBSGand epinephrine paste are available in Egypt and need simple instruments ( sensitive weighter and pharmaceutical mortar) to be prepared • BSG and epinephrine paste as a haemostatic method has the advantage of being atraumatic method·. This property attracts our attention to the anticepated benefits of the use of this paste. • In order to determine its advantages • we designed a porspective randomized study to compare between BSG as a haemostatic method and ligature and suture for haemostasis • Our study included 40 children aged between 8 and 10 years underwent tonsilllectomyfor reccurrent chronic tonsillitis. Twenty patients underwent tonsillectomy with BSG and epinephrine paste and the other twenty patients underwent tonsillectomywith ligation and suturetechnique • During the sugrery • application of BSG and epinephrine paste resulted in rapid coagulation of blood with the result of 54 swnmaTl/ and Conclusion relatively bloodless field which helped rapid easy and fine dissection . we also noticed that haemostasis by BSG and epinephrine paste was easier than ligation and suture which is a very importatnt advantage for the beginner surgeon • The application ofBSG and epinephrine paste resulted in reduction of operative time by ( 37 .2 % ) . This is important in high - risk patient and very small children in whom anaethesia time are of primary concern . • The application ofBSG and epinephrine paste resulted in reduction of operative blood loss by (23.1 %) • Decreasing operative blood loss is useful for the beginner ~ obtain relatively bloodless field and easy fine dissection . • The application of BSGand epinephrine paste resulted in decreasing of postoperative pain by (43.7 % ) . Decreasing post _ operative pain encourged the patients to begin eating of soft food immediately after recorery from anaethesia and made pain to be tolerable even without analgesic . • The effects of acute bismuth metal intoxication which include gastro-intestinal disturbances skin reaction and characteristic blue line on the gums did not appear in our group. 55 summary and Conclusion • BSG and epinephrine paste for haemostasis proved to be better than ligation and suture in reducing operative time operative blood loss and postoperative pain . • We conclude from the previous findings that BSG and epinephrine paste for haemostasis has several benefits for the patient and surgeon so we advice its wide use . |
