Abstract Pain is one of the major clinical problems confronting health care professional in general and those working in acute and postoperative setting in particular. Also pain is important stressors for critical care patients. Many sources of pain have been identified such as surgery, trauma, invasive equipment, nursing and medical intervention, and immobility. Accurate assessment of pain is crucial for efficient postoperative pain relief. Studies continue to show that many nurses neither assess pain systematically nor document their assessments our interventions when they complete them. Therefore introducing pain assessments into documentation can be helpful to make pain visible to all health care providers and helps to track patients` progress and the effectiveness of interventions. |
