Abstract

Background : Successful prevention and treatment of periodontitis is be influenced by effective control of the periodontopathic microbiota. Periodontal pathogens persist in deep subgingival plaque but also colonize supragingival plaque, tongue dorsum and further oral sites. Mechanical debridement is counted as the conventional technique in the management of chronic periodontitis, the locally delivered antiseptic agents used as an adjunctive therapy can help destroy periodontal pathogens in periodontal sites and in addition to SRP. Aim : The aim of this study is to evaluate the clinical and microbiological effectiveness of subgingival irrigation with 10% povidine-iodine and 0.1% hexetidine as an adjunctive treatment to (SRP) of patients with moderate chronic periodontitis. Subjects and Methods : Forty subjects were included in the study ; twenty subjects as healthy control and twenty Patients with moderate chronic periodontitis as study groups. Full-mouth clinical periodontal parameters, including periodontal probing depth, plaque index, gingival index and gingival bleeding index were recorded at baseline for study and control groups and after 5 weeks of treatment for study group. Plaque samples were collected at baseline at baseline for study and control groups and five weeks after therapy for study group. Plaque samples were cultured semi-quantitatively for detection of Prophyromans gingivilis and Prevotella intermedia. Result : At base line significant difference was found in all clinical indices and bacterial count, Prophyromans gingivilis and Prevotella intermedia between study and control groups. The results of the present study showed a significant improvement in all clinical parameters (GI, PI, PPD, PBI and CAL) after treatment in both sides of the study group. The side treated by SRP and PVP-I showed significant in all clinical parameter after treatment compared to side treated by HEX and SRP while there is no significant difference in all bacteriological analysis between both sides of the study group after treatment. The count of P. gingivilis, P. intermedia and total bacterial count were significantly reduced after treatment in both sides of the study group with no significant difference between both sides. Finaly ; after treatment significant difference was found in all clinical indices between study and control groups while there is no significant difference in all bacteriological analysis between study and control group. Conclusions : Subgingival irrigation with 10% PVP-I and 0.1% HEX is an effective adjunctive therapy to (SRP) in treatment of moderate chronic periodontitis, with more benefit derived from the use 10%PVP-I. Recommendations : We recommend after this study for additional longitudinal studies are needed to evaluate clinical and microbiological effect of adjunctive us10% PVP-I and 0.1%HEX to scaling and root planning(SRP) in treatment of patients with moderate chronic periodontitis.
