Abstract The current study was directed to estimate the role of talin-1, midkine and total antioxidant capacity (TAC) as tumor markers in HCC in comparison with cirrhotic patients and normal control subjects. The study included 75 patients, forty-four HCC patients and thirty one cirrhotic patients. In addition, 15 healthy individuals were included in the study. Serum talin-1 level was significantly decreased in HCC group when compared with cirrhotic and normal groups. In addition, cirrhotic patients exhibited significantly lower talin-1 concentration than normal controls. Talin-1 levels in HCC patients with portal vein invasion, metastasis and high BCLC score were significantly lower than those of patients without invasion, with localized tumors and at stages (A, B) of BCLC score. Significant negative correlation was found between serum talin-1 level and total bilirubin concentration in HCC group. Serum midkine was significantly increased in HCC patients in comparison with cirrhotic patients and healthy control. Furthermore, midkine level in liver cirrhosis was significantly higher than control group. Total antioxidant capacity was significantly decreased in HCC patients when compared to the control individuals but there was no significant difference when compared with cirrhotic patients. Based on the previous results, it can be concluded that: Talin-1 may be a promising diagnostic marker for HCC with better sensitivity and accuracy than AFP. It may be used as a marker for the severity and aggressiveness of the tumor as it correlated with invasion and metastasis of the tumor. Midkine may serves as a novel diagnostic marker for HCC with better sensitivity and accuracy than AFP. The combined determination of midkine and AFP increase the sensitivity of HCC detection. The significant reduction of TAC in HCC patients indicates that the antioxidant system of HCC patients was seriously impaired, and HCC tissues subjected to severe oxidative stress. The decrease of TAC in HCC patients suggested the need of antioxidants to aid in the protection from HCC. These findings should be validated in further prospective studies recruiting large number of patients for the clinical use of Talin-1 and Midkine in terms of early diagnosis and prognosis of HCC. |
