Abstract Toxic goiter involves : Primary toxic goiter “Grave’s disease” and Secondary toxic goiter “Plummer‘s disease” and Toxic nodule, They are enlarged thyroid gland that contains a small rounded mass or masses called nodules in 2ry type and diffuse enlargement of the whole gland in the 1ry type, which produce too much thyroid hormone. • Patients and Methods: This prospective randomized controlled study was conducted at general surgery department, Endocrine surgery unit at MUH. Sample size was calculated using G- Power with effect size 0.8 and power of 0.8 and it was 84 patients (42 patients in each group). 42 patients underwent total thyroidectomy (group TT), the other 42 underwent near total thyroidectomy (group NTT). The study was conducted between February 2018 and January 2019. • Inclusion criteria: Patients with toxic nodular goiter and patients with Grave’s disease with controlled toxic manifestations, Both Genders, Age was between 25 and 75. • Exclusion criteria: Patients with other thyroid lesions e.g.: (Cancer or Thyroiditis), Uncontrolled toxic manifestations, Patient with vocal cord problems All patients were submitted to: Full medical history, General and local examination, Radiological, Laboratory, Cytological investigations, Investigations were done to all patients like: Thyroid function tests (Serum T3, T4 and TSH). CBC, INR, Liver function tests and Serum Creatinine to assess effect of disease on it & as pre-operative preparation , E.C.G , Neck U/S & Duplex: Special probe to give idea about the nodule, (solid, cystic, size, homogenous, heterogeneous, well defined, ill defined, vascularity, infiltration to surrounding structures and retrosternal extension), Preoperative F.N.A.C (cytological examination), Post-operative pathology. |
