Abstract

Hysterectomy is a common gynecological procedure worldwide for benign uterine disease. Traditionally, this has been via the vaginal, laparoscopic, or abdominal routes. Despite the preference of ACOG and AAGL to VH as the first approach for benign uterine pathology, TLH is considered preferable alternative to open hysterectomy when VH is not feasible or indicated.We conducted a randomized prospective study in 55 women with different uterine diseases admitted to Mansoura University Hospitals for hysterectomy. We compared the perioperative outcomes between 28 women (group A) underwent early uterine artery ligation at start of TLH and 27 women (group B) underwent conventional TLH with respect to operative time, estimated blood loss and early postoperative complications.We realized that group A (early UA ligation) had a significantly shorter operative time (46.07 ± 6.83 vs 0.26 ± 11.38 minutes; P < 0.001) and less blood loss (66.57 ± 5.04 vs 91.44 ± 15.98 ml; P < 0.001) when compared to group B (conventional TLH). There was no statistically significant difference regarding early postoperative complications.
