Background: It is a dental disaster for child to lose his primary teeth due to caries before the age of normal exfoliation. The major role of pediatric dentistry is to maintain the primary teeth till the permanent successors erupt. Aim: of this study was to evaluate the efficacy of silver diamine fluoride on atraumatic restorative technique and incipient carious lesions of primary molars. Methods: This study conducted on thirty children aged 3-6 years old of both genders. Each child has bilateral lower first primary molars with proximal cavitated caries and bilateral second primary molars with proximal incipient non-cavitated caries. Pre-operative periapical x- ray were taken for both sides. Gross debris from cavitation of first primary molars were removed. Carious dentin on walls were excavated. Children were divided into two groups, group 1: ART technique and group 2: SMART technique. For group 1, GIC was applied. Lower second molar was polished with polishing brush. For group 2, protective coating was applied to the lips and skin. SDF was applied carefully using bended micro sponge brush. SDF was applied directly to only the affected tooth surface of lower first primary molar and the initial caries at the proximal surface of lower second molar. The lesion was dried for 15 seconds with gentle flow of compressed air. After one week GIC was applied. Clinical and radiographic evaluation were done for all teeth at 3, 6, 9 months. The data were collected then statistically analyzed using chi-square test to show the difference between groups. Results: After 3 months follow up, both techniques showed 100% success of all studied groups, clinically and radiographically. The restoration of the first primary molar with SMART technique showed higher clinical and radiographic success rate when compared to the restoration with ART alone with percentage of (83.3%, 76.7%), (70.0%, 56.66%) and (80.0%, 76.66%), (66.66%, 56.66%) after 6, 9-months of follow-up respectively. While, the second molars treated with SDF technique scored higher clinical and radiographic success rate than those treated with ART with percentage (93, 3%, 90.0%), (90.0%, 76.7%) and (86.7%, 76.7%), (80.0%, 63.3%) after 6, 9-months of follow-up. There was no statistical significance difference between all groups except for both techniques in second molars at 9 months follow up clinically and radiographically. Conclusion: SDF is successful in arresting caries and increase efficacy of ART technique in primary molars.
